1. The promoter of this “Spin to Win” sweepstakes promotion (“Promotion”) is U.S. HealthConnect, Inc. (“USHC” or “us” or “we” or “our”), which owns and operates USHealthConnect.com and the ReachMD network of websites, including but not limited to ReachMD.com (“Sites”), as well as the ReachMD mobile applications (“Apps”). This Promotion is not offered in affiliation with ReachMD supporters, providers, and/or educational partners.
2. Employees of USHC or their family members or anyone else connected in any way with the Promotion or helping to set up the Promotion shall not be permitted to enter or participate in the Promotion.
3. There is no entry fee and no purchase is necessary to enter this Promotion. To enter the Promotion and receive a prize, each entrant must provide their email address, and participation in the Promotion is limited to one “spin” per email address. Entrants who appear to be using alternate email addresses for multiple “spin” attempts are subject to disqualification.
4. Closing date for entry is indefinite until the Promotion is removed from USHC Sites or Apps. After this date, no further entries to or participation in the Promotion will be permitted.
5. No responsibility or liability can be accepted for Promotion entries that are not received by USHC for whatever reason.
6. USHC reserves the right to cancel or amend the Promotion and these terms and conditions without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside of USHC’s control. Any changes to the Promotion will be notified to entrants as soon as possible.
7. USHC is not responsible for inaccurate prize details supplied to any entrant by our sweepstake vendor or any other third party connected with this Promotion.
8. The prizes in this Promotion consist of Amazon gift cards ranging in value from $5 to $250 each. No cash alternative to the prizes will be offered. The prizes are not transferable. Prizes are subject to availability and we reserve the right to substitute any prize with another of equivalent value without giving notice.
9. Winners will be chosen at random by software provided by our sweepstake vendor from all entries received and verified by USHC and or its agents.
10. Any winner will be notified by email within twenty-eight (28) days of the date the individual has claimed any applicable CME/CE credits. If the winner cannot be contacted or does not claim the prize within fourteen (14) days of notification, we reserve the right to refuse the awarding of the prize to the notified winner.
11. USHC will notify the winner of any relevant details as to when and where the prize can be collected.
12. USHC’s decision in respect of all matters to do with the Promotion will be final and no correspondence will be entered into.
13. By entering this Promotion, an entrant is representing and warranting that they are eighteen (18) years of age or older and acknowledge that they have read, understood, and agree to be bound by these terms and conditions without modification. The Promotion is not available to anyone who is not legally eligible to be bound by these terms and conditions. Anyone who does not or cannot agree to be so bound is not authorized to participate in the Promotion.
14. The Promotion and these terms and conditions will be governed by U.S. law and any disputes will be subject to the exclusive jurisdiction of the courts of the U.S.
15. The winner agrees to the use of his/her name and image in any publicity material. Any personal data relating to the winner or any other entrants will be used solely in accordance with current U.S. data protection legislation and will not be disclosed to a third party without the entrant’s prior consent.
16. Entry into the Promotion will be deemed as acceptance of these terms and conditions. Void where prohibited.
