This process can be daunting, especially if patients are not ready to take control of their health care. However, with the right communication and preparation, the transition can be smooth for the patient, parent, and physician. Host Dr. Adam Ehrlich joins Dr. Philip Stein, a pediatric gastroenterologist at St. Christopher’s Hospital for Children, to discuss how physicians can best prepare patients to transition and overcome potential difficulties.
