In a randomized controlled trial,Bifidobacterium infantis EVC001 restored the breast milk-adapted gut microbiome in exclusively breastfed infants aged 2-4 months by reintroducing dominant, HMO-adapted bifidobacteria that were depleted in high-resource settings.

The trial randomized breastfed infants to high, medium, low, or placebo dosing arms and collected stool before, during and after supplementation to measure Bifidobacterium abundance and community composition.

B. infantis encodes pathways to metabolize HMOs and thereby outcompete other bacteria in the breastfed gut. When supplementation occurred alongside continued breastfeeding, B. infantis expanded and integrated into the resident community rather than passing through transiently. Serial stool sampling showed enrichment of HMO-metabolizing genes and sustained increases in bifidobacterial counts, supporting a biologic mechanism for durable colonization in the breastfed infant.
