Medical Creators Produce Higher-Quality Alopecia Areata Content on Social Media: Study

A cross-sectional study of alopecia areata content on 3 major Chinese social media platforms found that video quality, reliability, and adherence to clinical guidance varied substantially by platform and creator background.
Investigators evaluated 111 high-engagement educational videos identified on Douyin, RedNote, and Bilibili between March and May 2025. Two board-certified dermatologists independently assessed videos using the Global Quality Score (GQS), Journal of the American Medical Association benchmark criteria, and modified DISCERN. Content was also evaluated for adherence to alopecia areata guidelines.
Medical Creators Linked to Higher-Quality Alopecia Areata Videos
Of the included videos, 40 came from Douyin, 44 from RedNote, and 27 from Bilibili. Pathogenesis was discussed in 70.3% of videos and treatment in 64.9%, whereas clinical manifestations appeared in 27.0% and diagnosis in only 4.5%.
Platform differences were pronounced. Guideline-conforming videos accounted for 85.0% of content on Douyin and 81.8% on RedNote compared with 33.3% on Bilibili. Low-quality videos, based on GQS classifications, represented 47.5%, 59.1%, and 74.1% of videos on Douyin, RedNote, and Bilibili, respectively.
Creator background was also associated with video quality. Medical professionals produced 41 videos and non-medical creators produced 70. Medical creators had higher median GQS, JAMA, and mDISCERN scores, with all between-group comparisons reaching P < .001. Engagement metrics, however, showed inconsistent associations with quality across platforms.
The authors noted several limitations, including selection of only highly engaged videos, the cross-sectional design, potential subjectivity in quality scoring, limited generalizability beyond Chinese-language platforms, and exploratory correlation analyses without adjustment for multiple comparisons.
“This study systematically examined AA-related videos on three leading Chinese social media platforms, assessing their content structure, quality, accuracy, and reliability, and identifying key factors associated with high-quality educational content,” the authors wrote. “These insights may help inform policymaking and support efforts to improve patient education, contributing to higher-quality content for both the general public and patients on social media.”
