External Suturing Technique May Enhance Back Contouring After Weight Loss

A recent case series published in Plastic and Reconstructive Surgery–Global Open describes the use of external quilting sutures, or a “hemostatic net,” as an adjunct technique for lower back contouring in patients following massive weight loss.

The report included 8 patients who underwent liposuction followed by placement of transcutaneous quilting sutures to improve contour definition in the lumbar region. The technique extends prior applications of the hemostatic net, which has been used in cervicofacial rhytidectomy and thoracic procedures to reduce postoperative hematoma and improve skin retraction. In this series, sutures were placed using 1/0 braided material and removed after 2 days.

Patients had an average lipoaspirate volume of 1431 mL (range, 450–2300 mL), and follow-up ranged from 6 to 20 months. No complications or need for revision procedures were reported during the observation period. Aesthetic outcomes were assessed through pre- and postoperative photographic comparisons by both the patient and the surgeon, with reported improvements in lower back contour. Transient skin markings were noted but partially resolved by 6 months and were no longer visible at 12 months.

The authors note that the technique is intended to enhance contour shaping while minimizing scarring in a population often challenged by poor skin quality after weight loss. However, they emphasize that the findings primarily support feasibility and safety, rather than demonstrating superiority over existing contouring approaches.

“This technique offered an additional option in the plastic surgeon's arsenal to address massive weight-loss sequelae despite poor skin tone,” the authors wrote.
